Online Applications
The Ability Center is always interested in consumers participating in the activities of the Center.
Please feel free to use the applications listed below to become involved. All applications are accepted on a first come, first serve basis.
ADA Zoo Day Application. Sign up to participate in The Ability Center's annual event at the Toledo Zoo between May and June.
Volunteer application. Involve yourself in Center activities, from letter writing campaigns to online research to working with Camp Cricket and Youth Leadership.
Board Nomination Application. Nominate a responsible community member to ACT's Board of Trustees.
Scholarship Application. Apply for a scholarship to attend a local trade school or college.
Home Accessibility Program In-Take Application. Start the process of applying for a ramp installation or bathroom modification. If you meet geographic and income guidelines, you may qualify for financial assistance in getting a ramp installed, or a bathroom modified for better access.
